Holiday Food Basket Program Request Form

Community Action Partnership of San Bernardino County Food Bank is thankful for the ability to support numerous requests from food pantries and community partners each year for Thanksgiving and Christmas. Approximately 45,000 unique families and households are assisted each month throughout the county, because of the generous support food donors and the selfless work of our food distribution partners. However, the holiday time often exemplifies the struggle and food insecurity experienced by families and individuals residing in San Bernardino County. 

 

Each year, CAPSBC Food Bank receives a substantial influx of requests for support during the holidays. CAPSBC food bank works to fulfill as many requests as it is able, however every request received may not be approved. All holiday requests must be submitted using the electronic request form on this page, and only one request submission is needed per organization. The following information notes CAPSBC Food Bank’s priority and holiday distribution requirements:

 

Basic Requirements*

  • Organization must be a 501(c)(3) organization exclusively serving low-income individuals and families residing in San Bernardino County.
  • Event advertisement materials must be provided with the request.
  • Approved organizations must complete an agency program agreement form and adhere to the defined requirements.
  • Organizations must provide CAPSBC resources at no-cost to the community and the food distribution must be open to the public
  • Organizations with approved requests must submit completed distribution forms to CAPSBC Food Bank within 5 days after the distribution or event. Delayed or missing paperwork will affect future support of requests.
  • Recognition: Each organization with an approved holiday event request is asked to give recognition to Community Action Partnership for the support. For example: CAPSBC logo included on event materials, social media posting(s), etc.

*Please note that submitting an application and meeting program requirements does guarantee approval oy your request.

 

Request Approvals

CAPSBC Food Bank maintains discretion over approval of requests. Priority is based upon CAPSBC Food Bank’s determination and suitability.

Approval Process

Organizations receiving approval of a submitted request will receive an email notification verifying this approval and detailing next steps.
